Make a massage appointment

You may call or text me at 937-304-4301; please leave a message if I am not available. Messages left after normal business hours (M-F, 9-6), will be returned the following business day. My hours are M-F, days and late afternoons. No weekends or appointments later than 5PM.

First time clients - to save time, please print and fill out the following forms and bring them with you. If you forget them, I  will have copies in the office. Click here for the forms.  If you need to fill out the forms at the office, plan to arrive 10 minutes early so that the paperwork time won't take away time from your massage.

If you need to cancel or reschedule your appointment, I require 24 hours notice or you will be charged the full price of the massage. Please be aware - if you are late for your appointment, you will still be charged the full price for the massage; I will try to accommodate you by extending the massage to match your desired massage length, but I may be unable to do so due to other scheduled appointments.
